ABSTRACT

Objective:To discuss influence of addition and subtraction therapy of Zhenwutang to residual renal function (RRF), nutritional status, dialysis adequacy and quality of life of patients with maintenance hemodialysis (MHD). Method:One hundred and thirty-six patients were randomly divided into control group (68 cases) and observation group (68 cases) by random number table. Patients in two group got MHD, 3 times/week, 4 h/time, levocarnitine injection (1 g dissolved in 5-10 mL water for injection) after the dialysis, 2-3 min/time, recombinant human erythropoietin injection with subcutaneous injection for 4 weeks, 3 000 U, 3 times/day, valsartan capsules for 3 months, 80 mg/time, 1 time/day. The control group took Manshenning mixture, 35 mL / time, 3 times / day.Patients in observation group added addition and subtraction therapy of Zhenwutang for 3 months, 1 dose/day. Before and after treatment, urea nitrogen (BUN), creatinine (CR) and 24 hours' urine volume were recorded. And RRF, rate of decrease of RRF and rate of decrease in urine volume were also calculated. Levels of hemoglobin (HB), albumin (ALB), prealbumin (PA) and transferrin (TRF) were detected. After treatment, standardized protein metabolism rate (nPCR), urea clearance index (Kt / V) and glomerular filtration rate (EGFR) were discussed. And improved subjective comprehensive nutrition assessment (SGA), dialysis related quality of life (kdta) and health survey summary (SF-36) were graded. Six months' follow-up, primary end point event (24 h urine volume ≤ 400 mL) and no residual renal function (24 h urine volume ≤ 400 mL) were recorded. Result:Levels of RRF, Kt/V, nPCR, eGFR, Hb, Alb, PA, TRF and total scores of KDTA and SF-36 in observation group were higher than those in control group (P<0.01). And score of SGA, rate of decrease of RRF and rate of decrease in urine volume were less than those in control group (P<0.01).Incidence rate of primary end point event was 27.94%(19/68) lower than 47.06%(32/68) in control group (χ2=5.302, P<0.05), incidence rate of no residual renal functionwas 17.65%(11/68) lower than 36.76%(25/68) in control group (χ2=6.274, P<0.05). And BUN and Cr were lower than those in control group (P<0.01), 24 h urine volume was more than that in control group (P<0.01). Conclusion:Addition and subtraction therapy of Zhenwutang can maintenance of RRF, improvement of nutritional status, improvement of dialysis adequacy and quality of life of patients .

ABSTRACT

<p><b>OBJECTIVE</b>To establish DNA microarrays-based microRNA (miRNA) expression profiles of squamous cell carcinoma of larynx, using archived formalin-fixed paraffin-embedded tissue blocks, and to screen out and identify the differentially expressed miRNAs associated with the biological characteristics of this malignant disease.</p><p><b>METHODS</b>Total RNA was prepared from the formalin-fixed paraffin-embedded tissue blocks. After quality identification and fluorescent labeling, the RNA samples were hybridized with the Agilent human miRNA microarrays which contains 723 probes for human miRNAs. The data was processed with the softwares GeneSpring GX and R-Project.</p><p><b>RESULTS</b>From the formalin-fixed paraffin-embedded tumor blocks collected, 24 RNA samples were obtained with the quality accorded to the requirement of miRNA microarray analysis, and both the hybridization and consequent data processing were accomplished. A total of 319 miRNAs were identified and among them 96 were detected in all the 24 formalin-fixed paraffin-embedded blocks of laryngeal carcinoma; and 5 differentially expressed miRNAs (false discovery rate < 0.05) were found to be associated significantly with the lymphatic metastasis of laryngeal squamous cell carcinoma (P < 0.05), including miR-23a(*), miR-28-5p, miR-15a, miR-16 and miR-425.</p><p><b>CONCLUSIONS</b>Histopathological archives of well-annotated formalin-fixed paraffin-embedded tissue specimens are the valuable resources for miRNA study including to collect RNA samples for miRNA microarray analysis. A panel of differentially expressed miRNAs (miR-23a(*), miR-28-5p, miR-15a, miR-16 and miR-425) derived from the miRNA expression profile may serve as the potential molecular biomarkers for the prediction of metastasis development in laryngeal squamous cell carcinoma.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To study the clinical characters and the mode of treatment, and to analyze the prognosis of patients with sinonasal adenoid cystic carcinoma.</p><p><b>METHODS</b>The clinical data were analyzed retrospectively in 40 patients with sinonasal adenoid cystic carcinoma. The characters of survival rate, local recurrence and distant metastasis in these patients were analyzed using Kaplan-Meier method and Log-rank test.</p><p><b>RESULTS</b>The maxillary sinus was the most common site of origin and account for 80% of all patients. Five-year and ten-year overall survival rate were 76.9% and 61.6% respectively. Five-year and ten-year disease-free survival rate were 44.2% and 23.0%. Five-year local recurrence rate and distant metastasis rate were 45.0% and 23.0%, respectively. The most common site of distant metastasis was lung. Presence of distant metastasis correlated with decreased 5-year survival (chi2=7.26, P=0.007). The distant metastasis rate of preoperative treatment (18.2%) seemed lower than that of postoperative treatment (38.9%), but there was no significant difference in the two groups (chi2=1.37, P=0.24). The combined therapy mainly composed of surgery was adopted to treat the local recurrence. Five-year survival rate after development of local recurrence was 60.0%.</p><p><b>CONCLUSIONS</b>Distant metastasis is an important factor affecting the prognosis of the patients with sinonasal adenoid cystic carcinoma. Combined therapy composed of surgery and radiation was the first choice of treatment to the patients with sinonasal adenoid cystic carcinoma. Radiation can play an important role in the treatment. The combined therapy mainly composed of surgery should be adopted in the patients with local recurrence to improve the survival rate.</p>
